Job Description Facilitates patient admission, discharge, intra-hospital and inter-facility transfer processes through clinical triageActs as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the organizationProvides clinical expertise, close medical communication and quality assessment for the successful referral and transfer of patients to the facilityJob Responsibility Promotes optimum utilization of hospital beds; conducts rounds and meets with staff regarding bed availability and current censusAssigns patients to available beds to avoid delays in patient arrival to unitsCommunicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely mannerAssists with and ensures admission paperwork is complete, and ensures hand-off of communication to accepting nurseEnsures beds are assigned based on level of care and patient aggregation; addresses and resolves issues arising from increased census and decreased bed availabilityPromotes patient, family and physician satisfaction regarding bed assignments; addresses and resolves conflict regarding dissatisfaction with bed assignmentsAddress any conflicts or issues arising from dissatisfaction with bed assignmentsEnsures patient information and data entry in patient's chart is accurate, including but not limited to insurance information, clinical documentation, transportation requests, and patient demographicsEvaluates, processes, tracks, documents, and coordinates patient transfers for the successful referral and transfer of patients to the facility; may conduct recorded three-way calls between referring physician and facility physicianPerforms related duties as requiredAll responsibilities noted here are considered essential functions of the job under the Americans with Disabilities ActDuties not mentioned here, but considered related are not essential functionsJob Qualification Bachelor's Degree in Nursing (BSN) requiredCurrent License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed2-5 years of technical experience and 0-2 years of leadership / management experience, requiredCare Escalation Experience is preferred Adult Critical Care Experience is preferred Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
